Editor’s Note: Old, Reliable Resources

For better or worse, many patient safety resources hold their value over time. Hazards in healthcare persist, and many of the solutions we advocate today date back years or decades. I was reminded of this recently when two patient safety experts recommended the same two-part article on high-reliability organizations. The Institute for Safe Medication Practices (ISMP) published “High-Reliability Organizations: What They Know That We Don’t” (parts I and II) in July 2005. The articles, written in clear, sophisticated, yet unfussy language seem timeless and are well worth reading.

Another reliable resource is the complete archive of articles published in PSQH. Each year, the Quality Colloquium marks the anniversary of our first issue, published in late summer of 2004. Everything we’ve published since is available online. The archive is available through the contents of each issue or search, and in digital editions for Nov/Dec 2008 up to the present.
