Award Recognizes Excellence in Patient and Family Engagement

IORA Health and the Children’s Hospital of Philadelphia (CHOP) Patient and Family Experience Team and Outpatient Clinical Service Leadership were the winners of the third annual John Q. Sherman Award for Excellence in Patient Engagement at the National Patient Safety Foundation’s (NPSF) 18th Annual Patient Safety Congress last month in Scottsdale, Arizona.

Iora Health was chosen for their transformative approach to patient-centered care focusing on building an engaging model of high impact relationship-centered care with a specific patient population – Dartmouth College employees. They restructured the Iora financing model and assigned personal health coaches to each patient and transformed their business model to one that truly addresses patient needs and desires. As a result, the team increased clinical quality; reduced use of expensive medical services (including hospitalizations and emergency department visits); and enhanced the quality of patients’ care.

CHOP received the award for the creation of a new ambulatory care building, called the Buerger Center Advanced Pediatric Care. A large portion of the population visiting the Center are diagnosed and treated for the Autism Spectrum Disorder. CHOP’s Patient and Family Experience Team and Outpatient Clinical Service Leadership involved patients and families in every step of the planning from design and planning to patient flow layout and construction of the Center.

The award was given by Taylor Healthcare and NPSF’s Lucian Leap Institute on behalf of EngagingPatients.org, the online community that sponsors the award. The Sherman Award recognizes innovative programs that are improving care and outcomes through patient and family engagement.
