Quality Conundrums
 

New Beginnings – A Quality Conundrum?

Traditional patient safety, risk management, and performance improvement plans rely heavily on Root Cause Analysis as a tool to evaluate adverse outcomes and avoid preventable recurrences of the same or similar events. Six Sigma/Lean methodologies require the dissection of existing processes to facilitate efficiencies and eliminate waste. These approaches seem to resonate with the philosopher Edmund Burke’s oft repeated quote, “Those who don’t know history are destined to repeat it.” Most of us in the quality biz seem to believe this.

We all also know that change comes slowly and in fits and starts. Analysis of adverse events, often painful, doesn’t always lead to actionable quantifiable change. Many of us wonder why. We struggle with fixing what we know needs to be fixed, and sometimes with the sense that we know something is very wrong, but we don’t know how to fix it. We leave post-adverse event discussions feeling hollow; pained and beaten, but without a clear path forward.

All of which leads me to wonder if the focus on the past is the best road to the future. As Benjamin Franklin once said, “How few there are who have courage enough to own their faults, or resolution enough to mend them.” For many of us, would letting go of the past enable us to more effectively envision a brighter future? Would decreasing the focus on what we have done wrong, with the attendant, though unintentional, blinders, and excuses enable us to see what needs to be seen more clearly?

As we move into a new year, consider whether leaving some memories behind will enable clear sightlines to a brighter, safer patient experience. Happy New Year!
