Quality Conundrums

Is Efficiency a Quality Conundrum?

Weekend inefficiency saved my cousin, which begs the question, “Is efficiency a quality conundrum?” As gentlemen of a certain age so often do, my cousin was climbing on his roof to retrieve a child’s ball and a newspaper. This time, his journey had an unexpected ending. He arrived on the ground without the benefit of a ladder. His wife did not allow him to move and called for help. Astute EMTs transferred him safely and quickly to the local university hospital center, and that is where weekend inefficiency saved him.

His pain was horrific, but the team that evaluated him was unimpressed. They read (or perhaps misread) x-rays and told him and his concerned family that all he needed was pain relief and a back brace; he could go home and would be fine. Unfortunately, the inefficient back brace provider could not make the brace available until Monday, so my cousin would have to remain in the emergency room until then, which he did.

Enter the trauma attending physician on Monday. He evaluated the patient fully, took a complete history, and became concerned that the severity of the injury had not been fully appreciated by others. An emergent MRI followed, and you can guess the rest. My cousin had a fracture and severe ligament damage and 
instability. He required urgent neurosurgical intervention. Hopefully, he will make a full recovery. Had things been more efficient, he would likely not have recovered at all.

Merriam-Webster defines efficiency as “the ability to do 
something or produce something without wasting materials, 
time, or energy: the quality or degree of being efficient 
(technical).”1 Apparently quality is not a consideration in 
some definitions of efficiency.

So please…let’s be careful! And as we are being efficient, 
let’s not forget to be thoughtful. Let’s not forget the patients whose lives are in our hands. And let’s not let efficiency 
(inarguably a good thing when coupled with quality care and 
compassion), become another quality conundrum.
