ThedaCare Selects Phytel to Strengthen Its Patient Engagement Process

ThedaCare Physicians, the clinic division of the ThedaCare healthcare system, has agreed to use Phytel to help the organization manage population health and secure National Committee for Quality Assurance recognition of its patient-centered medical homes. NCQA provides automatic credit to providers who deploy Phytel to meet certain elements of the medical home requirements.

ThedaCare’s employed primary care providers will initially use Phytel’s automated outreach solution to identify and notify patients with gaps in recommended preventive and chronic care. The healthcare system intends to expand its patient engagement solution to all of its 200-plus primary care providers.

The Appleton, Wis.-based healthcare organization views Phytel as a component of its overall strategy for population health management and promoting principles of process redesign. Phytel will be used to supplement ThedaCare’s disease registries and its electronic health record as the organization seeks certification for its patient-centered medical homes, builds its accountable care organization (part of Medicare’s Pioneer ACO program), and completes a five-year transformation of its primary care model.

We do really well in providing appropriate care to the patients who visit their providers regularly. Phytel can help us engage those people we haven’t seen in a long time and allow us to create a more robust approach to population management. We have our internal processes and our EHR, but if we want to really manage our population’s health, we’ll need additional tools like what Phytel offers.”
