Number of Adverse Health Events in Minnesota Hospitals Increases Slightly in 2011 PDF Print E-mail

The number of reportable adverse events in Minnesota hospitals, ambulatory surgical centers, and community behavioral health hospitals increased from 305 in 2010 to 316 in 2011, according to a report released by the Minnesota Department of Health (MDH). Despite the increase, the number of events resulting in serious injury or death to a patient decreased from 107 in 2010 to 89 in 2011.

Joint Commission Issues Alert on Healthcare Worker Fatigue PDF Print E-mail

The Joint Commission (TJC) has released Sentinel Event Alert 48, "Health Care Worker Fatigue and Patient Safety." Acknowledging that "many factors contribute to fatigue, including but not limited to insufficient staffing and excessive workloads," TJC states that "the purpose of this Sentinel Event Alert is to address the effects and risks of an extended work day and of cumulative days of extended work hours."

IHI Announces Pilot Program with CDC to Improve Antibiotic Use in Hospital PDF Print E-mail

Cambridge, Massachusetts—The Institute for Healthcare Improvement (IHI) announced the launch of a pilot testing program designed to avoid overuse and misuse of antibiotics in hospitals. Designed in collaboration with the Centers for Disease Control and Prevention (CDC), the program focuses on a new jointly developed initiative, the CDC/IHI Driver Diagram and Change Package for Antibiotic Stewardship.

IOM Issues Report on Patient Safety and Health IT PDF Print E-mail

Nov. 8, 2011—To protect Americans from potential medical errors associated with the use of information technology in patient care, a new report by the Institute of Medicine calls for greater oversight by the public and private sectors.  The report examines a broad range of health information technologies, including electronic health records, secure patient portals, and health information exchanges, but not software for medical devices.
